For PPO:
- $1,000 in-network deductible per person/year.
- $2,000 out-of-network deductible per person/year.
- Co-insurance employee responsibility — 20% for in-network services, 20% for out-of-network services.
- In-network, out-of-pocket maximum after deductible $3,000 single and $5,000 family.
For HDHP:
- $2,000 in-network deductible per family/year.
- $6,000 out-of-network deductible per person/year.
- Co-insurance employee responsibility — 0% for in-network services, 40% for out-of-network services.
- In-network, out-of-pocket maximum after deductible $2,000 single and $7,000 family.
Prescriptions:
- PPO: Retail Pharmacy (30-day supply) 30% coinsurance to a maximum of $100. Deductible does not apply. Mail order (90-day supply) 30% to a maximum of $200. Deductible does not apply.
- HDHP: Retail Pharmacy (30-day supply) 20% in-network and 40% out-of-network after deductible. Mail order (90-day supply) 20% in-network and 40% out-of-network after deductible.

The Health Savings Account allows you to redirect a portion of your earnings (pre-tax) to pay for qualified medical expenses that may or may not be covered under your insurance program. The total amount of your annual election is available upfront, reducing your chance of incurring a large out-of-pocket expense early in the plan year.
Employees selecting the HDHP Medical Plan are eligible for HSA enrollment. Kish provides an employer contribution to HSA members. New employees enrolling after Jan. 1, 2024, will receive a prorated deposit based on the first full month enrolled in the HSA/HDHP medical plan. See employer contribution below:

Plan Highlights:
Benefit-eligible employees receive $50,000 employer-paid Group Term Life Insurance coverage with no tax implications. Employees may elect a higher employer-paid Group Term Life Insurance in the amount of 1.5x their base salary in lieu of the $50,000 with employee-paid tax implications.
Supplemental Life AD&D Insurance is available for employees, spouses and dependents. Benefits cannot exceed 7x the annual salary or $750,000, whichever is less.
